Event Information
If you are bringing an outside guest, you must complete an outside guest form.
The guest form MUST be filled out and turned in before purchasing your guest's ticket.

Only current juniors and seniors may attend Prom. If you are bringing an outside guest
who is a college student, they must be under 21 years of age.

The form below must be filled out and returned to the office by Friday, April 25.
This must be filled out for anyone who doesn't attend St. Dominic.

2025 Prom Visitor Dance Form

Only students who purchase Prom tickets will be eligible for Prom court.
Please remember to have the QR code available for check-in at the event.
